Some examples: Improved or new housing, adult education/training, self-employment initiatives, budgeting, effective parenting skills, etc.
o Through kanohi ki te kanohi in the comfort of your own home we will provide professional advice & guidance and will walk with your whanau to support you achieve your goals
o Link your whanau to external agencies and/or services that can further assist you to achieve your whanau development goals
o If you require special support we can go with you ti visit any external /agency service.
o Supporting you in any way you would like us to support you when talking to people who work in these services
o Providing more information about each and any of these agencies and/or services
o Under exceptional circumstances we will provide transport to/from an external service/agency.
Hapū Development:
o Through Luncheon Hui on your Marae we will help your hapu identify its collective dreams & aspirations (hapu development goals)
Some examples: Marae development, hapu employment initiatives, new and/or improved hapu facilities, etc.
Who is responsible for helping me?
Your Service Manager is the person who will provide you with the support, advice & guidance you need for your whanau & hapu development projects.
However, please feel free to discuss your dreams & aspirations with your Whanau Ora Kaimahi during one of her regular visits to your home – especially when you are discussing and/or revising your whanau ora Care Plans.
How do I ask for help?
o Telephone your Service Manager at our administration office and ask for a special urgent home visit
Or,
o Talk to your Service Manager during a hapu health promotion hui on your Marae
Or,
o During one of your Whanau Ora Kaimahi’s regular home visits ask her to arrange an appointment for your Whanau Development Manager to visit you in your home

Some examples: Heart disease, lung disease, financial stress due to ever increasing cost of tobacco, etc.
o Assisting you to identify your smoking habits in preparation of quitting
o Providing you with subsidised nicotine replacement support over a period of time to help reduce the negative side affects of withdrawal
o Supporting you in any way you would like us to support you when talking to your whanau about your desire to quit smoking
o Provide you with caring kanohi ki te kanohi guidance and work with you towards ensuring a successful outcome for you

Your Whanau Ora Kaimahi has been given the professional training required to provide you with the National Heart Foundation’s Quit Programme.
She will ensure you receive the advice & guidance you need to both consider the pros and cons of quitting and thereafter support you kanohi ki te kanohi to become Auahi kore.

We can assist you by:
Health & Tamariki Ora:
o Listening to any concerns you may have and assist you work out a tamariki care plan to overcome any issues
o Being present to support you when the Tamariki ora Nurse holds a clinic at Jerusalem (available to Jerusalem residents only)
o Being present to support you when the Tamariki Ora Nurse holds clinics and/or visits you in your home (available to Pipiriki residents only)
o If you require special support we can go with you to visit any external service such as your: . Doctor . Specialist . Any other Health professional
o Supporting you in any way you would like us to support you when talking to people who work in these services
o Under exceptional circumstances we will provide transport to/from an external service
Immunisations:
Assist you with immunisation for your tamariki by:
o Tell you about the way vaccines work
o Explain the Informed Consent process
o Tell you about the National Immunisation Register
o Link you to a family Doctor or the Tamariki Ora o Nurse to complete all remaining immunizations
o Help you reduce the risk of your tamariki being admitted to hospital for whooping cough, measles, meningococcal B etc. o If you don’t know whether your tamariki are fully immunised we can find out for you by either talking with your Doctor or contacting the National Immunisation Register

Health promotion is an important part of whanau ora (family wellbeing) care and is one of the ways in which we support you to increase control over the factors that influence your health, quality of life and whanau ora.
In simple terms we provide you with healthy lifestyle choices which focus on ensuring wellness not illness, i.e. we provide you with ‘wellness’ information tools which you can use to prevent ‘illness’ from occurring.
What is a health promotion hui?
During your community luncheon we will provide you with health promotion hui on your Marae.
Most of the health promotion we provide at these hui are based on the national health promotion annual calendar primarily designed by the Ministry of Health – some examples: asthma control & prevention, healthy eating lifestyles, diet & exercise, auahi kore, etc.
However, we are also able to provide you with health promotion on other topics that you may want which are not part of the national health promotion calendar – some examples, rongoa & mirimiri, fire prevention, road safety & licensing, alcohol & drugs, mental health education.
Where & how often are health promotion hui held?
These are held on your Marae during your community luncheon and are usually held every two months.

How do I ask for a health promotion hui to be held on a topic of interest to me?
o Talk to your Whanau Ora Kaimahi during one of her regular visits to your home

o You can also ask your external provider to make a direct referral to your Whanau Ora Kaimahi
o Collectively discuss your hapu needs at your community luncheon and request your Hauora organise a health promotion hui to be held at your next community luncheon

o Supporting you in any way you would like us to support you when talking to people who work in these services
